Beginning on June 1, 2025, and ending on July 31, 2025, every customer who both purchases and installs a NATURESTONE® Brand Floor will be entered to win your choice of either a 9-Day Five Epic National Parks Tour from Jackson, WY to Las Vegas (“Epic Park Tour”), a 7-Night Key West Adventure at Ocean Key Resort & Spa (“Key West Adventure”) or a 7-Night Treehouse Adventure at Treehouses at River Ranch in Hocking Hills (including a $1,000.00 gift card) (“Treehouse Adventure”). These three prizes have an approximate retail value ranging from $4,500.00 to $9,100.00 or alternatively the Winning Customer can elect to receive a cash prize of $5,000.00, subject to IRS withholding requirements (The Epic Park Tour, Key West Adventure, Treehouse Adventure or the cash option shall be collectively referred to as “Promotional Prize”). The Promotional Prize is being provided to you by Cleveland Cruise and Travel. Ohio Concrete Resurfacing, Inc. (“Ohio”) does not endorse, support, sanction, or approve the Promotional Prize. OHIO MAKES NO REPRESENTATION OR WARRANTIES WHATSOEVER, EXPRESS, OR IMPLIED WITH RESPECT TO THE PROMOTIONAL PRIZE, INCLUDING THE WARRANTIES OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE IN WHICH THESE WARRANTIES ARE SPECIFICALLY DISCLAIMED.

The approximate retail value of the Promotional Prize is between $4,500.00 to $9,100.00 (“APV”). Any difference between the APV and actual prize value will not be awarded.

The Promotional Prize is non-transferable, and no substitution will be made except at Ohio’s sole discretion. Ohio reserves the right to substitute the Promotional Prize for one of equal or greater value. The winning Customer will be solely responsible for all federal, state, and local income taxes based on the value of the Promotional Prize and will be issued an IRS 1099 form. Additionally, the Customer shall be responsible for any and all applicable registration, license, taxes, title and insurance fees, state inspection fee and any expenses not listed herein related to receipt, possession, acceptance, titling, and use of the Promotional Prize.

The actual Promotional Prize may vary from any depictions of it in any promotion and/or advertising materials. The Winner Customer will be notified using the contact information entered on the Customer’s Purchase Agreement within three (3) business days from the drawing and will be notified that they were selected as the Conditional Winner of the Promotional Prize and will be conditionally awarded the Promotional Prize. If Ohio is unable to contact any Conditional Winner or the Conditional Winner fails to respond to Ohio’s initial contact within three business days of notification the Conditional Winner will be disqualified and forfeit the prize and an alternative winner will be notified as the new Conditional Winner.

The Winning Customer by accepting the Promotional Prize consents to Ohio and its agents use of Winning Customer’s name, likeness, photograph, voice, opinions and/or hometown and state for promotional purposes in any media, worldwide, without further payment or consideration and as a condition precedent to receiving the Promotional Prize, shall sign all necessary waiver(s) and document(s) relating to this paragraph.

Further, the Winning Customer by accepting the Promotional Prize hereby grants to either Ohio and/or O.C.R. Products, Inc. a worldwide, royalty-free, perpetual, unrestricted rights, and license to take photos and/or videos of the Winning Customer’s NATURE STONE® floors and the homes that they are installed in and to use those photos or videos on their website, on social media, in brochures and/or in any marketing campaign. Additionally, the Winning Customer consents to Ohio and its agents use of winner’s name, likeness, photograph, voice, opinions and/or hometown and state for promotional purposes in any media, without further payment or consideration. As a condition precedent to receiving the prize, the Winning Customer must execute a release and authorization to effectuate the conditions of this paragraph. The language of the release and authorization shall be at the sole discretion of Ohio Concrete Resurfacing, Inc. The failure to execute the release as required by Ohio Concrete Resurfacing, Inc. shall cause the prize to be rescinded in such case the Winning Customer shall not be entitled to receive that Promotional Prize.

The contest is open to all legal residents who are 18 years of age or older and who possess a valid driver’s license. The odds of winning depend upon the number of eligible entries received. Employees of either Ohio or O.C.R. Products, Inc. and their immediate family and other household members (i.e., spouses, parents, grandparents, children, grandchildren, roommates, housemates, significant others, partners, and siblings) of each of the above are not eligible to enter and/or to win the contest. Ohio shall be the sole arbitrator in all matters relating to the contest and in the interpretation of contest rules. Ohio’s decisions shall be final. Entry into the contests constitutes agreement by contestants to abide by these rules, as well as any other rules established by Ohio.

Each Customer who is entered into this Contest and Giveaway agrees to:

(a) Comply with and be bound by these rules and the decisions of Ohio shall be binding and final in all matters;

(b) Release and hold harmless Ohio, and each of their respective parents, subsidiary, and affiliated companies, the Promotional Prize suppliers and all of their respective past and present officers, directors, employees, agents and representatives (collectively, the “Released Parties”) from and against any and all claims, expenses, and liability, including but not limited to negligence and damages of any kind to persons and property, including but not limited to invasion of privacy (under appropriation, intrusion, public disclosure of private facts, false light in the public eye or other legal theory), defamation, slander, libel, violation of right of publicity, infringement of trademark, copyright or other intellectual property rights, property damage, or death or personal injury arising out of or relating the Customer’s entry, creation of an entry or submission of an entry, participation in the Contest and Giveaway acceptance or use or misuse of Promotional Prize (including any travel or activity related thereto) and/or the broadcast, exploitation or use of the Customer’s entry; and

(c) Indemnify, defend, and hold harmless the Released Parties against any and all claims, expenses, and liabilities, including reasonable attorney’s fees arising out of or relating to the Customer’s participation in this Contest and Giveaway and/or the Winning Customer’s acceptance, use and/or misuse of the Promotional Prize. Ohio shall not, in any way, be responsible for any product liability claim regarding the Promotional Prize.

(e) Your guests must execute a Liability Release and Publicity Release prior to the issuance of the travel documents. If a guest is under eighteen (18) years of age, a parent or a legal guardian must sign on behalf of such guest.

The drawing will be held in the corporate offices of Ohio located at 1955 Midway Dr. Twinsburg, Ohio 44087, phone 440-786-9100. Management will conduct the drawing. This Contest and Giveaway shall be construed in accordance with and governed by the applicable laws of the State of Ohio. All transactions and the Contest and Giveaway are deemed to take place in our main headquarters located at 1955 Midway Dr. Twinsburg, Ohio.

Any legal disputes shall only be brought in the Stow Municipal Court, the Summit County Court of Common Pleas, or the Federal District Court for the Northern District of Ohio. If Ohio accrues any type of expense, including, without limitation, reasonable attorney fees, costs and expenses, in defending or enforcing this Contest and Giveaway or any claim regarding the Promotional Prize, including but not limited to, negligence, product liability, tort and/or warranty, then OHIO shall be entitled to recover any and all such costs, expenses and attorney fees, to the extent permitted by law.

This contest is subject to all applicable laws and regulations and is void where prohibited. The drawing will be conducted in mid-August 2025, with the winner announced Labor Day Weekend, 2025.
